In the event of a medication error…

If a medication error occurs, follow these steps:
  • Notify the prescriber immediately.
  • Consult the pharmacist for information regarding drug interactions, solutions to dose-related problems such as what to do about an overdose or omitted dose, and an antidote, if needed.
  • Continue to assess the patient, paying close attention to the drug’s action and possible effects.
  • Follow facility policy for documentation. You may have to complete an incident report for legal purposes. If so, clearly document what happened without defending an action or placing blame. Record the names and functions of everyone involved and what actions they took to protect the patient after the error was discovered.
