Most elderly patients take several medications, often prescribed by different health care providers. Routinely review the names, dosages, frequency, and route of each medication being taken. A patient may be taking two medications that result in the same action. Always ask the patient or family member to bring all medications to the office or hospital so that a thorough medication review can be performed. |
